We are seeking a compassionate, proactive and organised Home School Support Co-ordinator to join our team.
This is an exciting opportunity to make a real difference to the lives of children with special educational needs and disabilites (SEND) and their families. You will work closely with parents, carers, school staff and external agencies to strengthen home-school relationships, improve attendance, support emotional wellbeing and help remove barriers to learning.
Guided by our values of Teamwork, Positivity, Courage, Respect, Independence and Curiosity, you will provide practical support to families, coordinate targeted interventions and support plans, deliver parent workshops, and contribute as a Deputy Designated Safeguarding Lead.
We are looking for someone with excellent communication and relationship-building skills, who is committed to helping children and families achieve the best possible outcomes.
We are seeking a candidate who can start in January. To support a smooth transition, the successful candidate will spend their first term working alongside the current Home School Support Co-ordinator, benefiting from a comprehensive induction and opportunities to build relationships with pupils, families and partner agencies.
Key responsibilities:
- Build strong relationships with children, families and professionals.
- Lead and implement family support programmes and interventions.
- Develop support plans with families and monitor progress against agreed targets.
- Deliver Parent Connect sessions and facilitate workshops on topics such as behaviour, sleep, toileting, eating and emotional wellbeing.
- Act as the link between the school and the named Community Paediatrician, coordinating appointments and ensuring effective communication between home, school and healthcare professionals.
- Deliver targeted interventions for pupils.
- Promote and improve pupil attendance across the school.
- Support parents and carers to access community activities, services and support networks.
- Attend and contribute to multi-agency meetings.
- Work collaboratively with teachers and support staff.
- Maintain accurate records, monitor the impact of interventions, and contribute to school improvement priorities.
